Immunization requirements

Day Care and Pre-School

Influenza Vaccine – Children age 6 months to 59 months (4 years 11 months) must get 1 dose of influenza vaccine each year to attend pre-school or day care between September 1st and December 31st.

Pneumococcal Vaccine – Children age 2 months through 11 months, enrolling in or attending any licensed childcare or pre-school facility shall receive a minimum of 2 doses of pneumococcal conjugate vaccine, unless medically contradicted with a note from the child’s primary care provider.

Children 12 months through 59 months of age enrolling in a licensed childcare facility must have received 1 dose of pneumococcal conjugate and 1 dose of HIB on or after the first birthday.

Sixth Grade

Tdap (Tetanus, Diphtheria, and Acellular Pertussis) – Children entering Grade 6 and age 11, or a comparable age level special education program or transferring into a new Jersey school from another state or country are required to receive a booster dose of Tetanus, Diphtheria and Acellular Pertissis (Tdap). Students who have received a DTP/DTap or Td less than 5 years prior to entry or attendance will not be required to receive a Tdap until 5 years have elapsed from the last dose.

Meningococcal – Children entering Grade 6 and age 11 or a comparable age level special education program with an assigned grade shall have received 1 dose of meningococcal containing vaccine.
