1.Register for your sport on Family ID - this must be done for every sport per season

2. Submit a valid physical to the Nurse AND Athletic Trainer - it must be completed on the state mandated forms. Once submitted, it needs to be approved by our school doctor. 

A physical is valid for 1 YEAR following the date of the physical exam

3. Complete a baseline concussion test on impact 

Instructions on how to complete a baseline impact test: 

Athletes need to have a laptop or computer to take the test- it CANNOT be taken on a phone or tablet.

Follow the steps below:
1.Navigate to the testing link below

2. Enter the Customer ID Code YP12FU43AF and click ‘Validate’
3. Click ‘Launch Baseline Test’ and begin the test
4. Once completed, let Mrs. T know so she can check that it is valid
